Patients have the right to select the Home Health Agency of their choice. To qualify, home health care patients must be under the care of a physician and must need medical services or personal care. For some payor sources, a patient must be homebound. This means leaving your home requires taxing effort.

Integrity Home Health LLC is a State-Licensed, Medicare Certified Home Health Care Agency. We provide home health care in the patient’s place of residence.
The agency is dedicated to providing high-quality comprehensive, individualized, and consistent care according to the physician’s plan of care and is covered by Medicare or other payers.
Our interdisciplinary team focuses their care on maximizing the patient’s functional abilities and improving the patient’s medical condition to the highest extent possible.
